You join the Scheme by filling out an enrolment form that asks you for your name, date of birth, address, the company that you are employed by, your position in the company and the date that you were employed by the company. The form also asks you to name your beneficiaries in case you die whilst in the employment of the company or within the first five or ten years of retirement.

Each employee is required to submit their birth certificate and the birth certificates of their beneficiaries (and trustees if applicable) with this enrolment form.
